Insurance companies with which we participate:

  • Delta Dental of all states within the Premier Provider Level
  • Delta Dental Premier PPO

**We will bill to ALL dental policies, with the exception of Medicaid, MiChild or Healthy Kids, regardless of their network participation, and accept their payment. However, our formula utilized in regards to Out Of Network policies includes an internal processing procedure to the patients’ balance due. This is done with the intent to cover the difference between our submitted charge(s) and the insurance allowance for the procedure(s). Should we receive an insurance overpayment as a result of this processing, we will issue a refund to the patient in the form of a check for this overpayment.

**We are able to submit a request for pre-treatment estimates (Pre-Determination), however, this procedure can typically take up to a month and does not result in a guarantee of payment from your insurance company.

Cancellation Policy

Kindly provide at least 48 hours notice for any appointment that needs to be rescheduled. Appointments cancelled in less than 24 hours may be subject to a cancellation charge.
